Frequently Asked Questions
If my dentist does not participate with United Concordia can I still see him/her?
Yes, you can receive care from any licensed dentist, anywhere in the United States. If you choose to see a non-participating dentist, you will be responsible for the out-of-network deductible and/or coinsurance amount, as well as any charges over and above Untied Concordia’s maximum allowance for covered services.
How do I find out if my dentist participates in the United Concordia network?
You can access provider network directory information by visiting United Concordia’s website www.ucci.com, Network: Concordia ADVANTAGE, or by calling our toll free Customer Service at 1-800-332-0366
What does “maximum allowable charge” mean?
The “maximum allowable charge” is the discounted fee that participating dentists agree to charge for a covered service. United Concordia network dentists accept this allowance as payment-in-full and collect only the applicable deductible and coinsurance from the members.
Does United Concordia require predetermination of benefits?
We do not require a predetermination. You should consider asking your dentist to request a predetermination of benefits before you begin treatment for any major procedure. If you utilize an out-of-network dentist, you should consider a predetermination of benefits before beginning any treatment. That way, you’ll know whether the service is covered and exactly what your financial responsibility will be.
Advantages of Utilizing Network Providers
All participating network dentists agree to:
Accept United Concordia’s maximum allowable charge as payment-in-full for covered services.
File claims with United Concordia
Abide by our utilization review decisions.
Dentists are monitored to ensure a high level of quality in the services they provide you.
When you receive services from a participating provider, you are responsible only for those deductibles and coinsurance amounts that are part of the program design.
There is no balance billing for covered services and no claims to file.
If your dentist does not participate with United Concordia:
You may have to file your own claim for reimbursement from United Concordia.
Non-participating dentists are not bound by any agreement. If your dentist is not a participating dentist, you may need to pay the difference between the allowance and the dentist's usual charge.
About United Concordia
United Concordia was founded in 1992 for the sole purpose of insuring and administering dental products and programs. As the fifth largest dental carrier in the country, United Concordia covers more than six million members and offers a national network of more than 55,000 participating dentists, including 1,400 in Oregon and Washington .
A major part of what makes United Concordia unique is the delivery of dental benefit programs through contracted networks of dentists. Locally contracted networks enable us to deliver access and savings to our customers and members. All contracted providers agree to accept our allowances as payment in full with no balance billing to United Concordia members.
United Concordia uses some of the largest and most sophisticated dental systems in the administration of membership, claims and customer service. This level of automation, coupled with our focused dental expertise, creates an unbeatable combination for our customers.
Our customers include large national companies, small employers; federal, state, and local government entities; medical HMOs and major insurers. No matter the size of the industry, United Concordia provides solutions to our customers' dental insurance needs. Because our market scope is limited exclusively to dental programs, we are more readily able to address the dental service needs of our customers and provide more cost effective products.
